1. What is exactly happening when teeth are getting yellow? 

 Tooth staining can be caused by many local and systemic conditions.  Tooth stains can be either “intrinsic” or “extrinsic.”  Intrinsic stains are stains located within the tooth structure.   They are caused by such things as dental cavities; various dental materials, such as are used in some restorations and in the treatment of certain disorders, including, among others, complications of pregnancy, bleeding disorders, bile duct problems, and genetic defects; and hereditary diseases that affect enamel and dentin development or maturation.  Most often the appearance of deep intrinsic stains can be remedied only through use of non-vital bleaching procedures to lighten the stains, or cosmetic restorations, such as crowns or veneers, to mask them. 

 

Extrinsic stains are defined as stains located on the outer surface of the tooth structure.  Extrinsic stains are categorized under the Nathoo classification system as follows:

 

  • Nathoo type 1 (N1): N1-type colored material (chromagen) binds to the tooth surface. Examples of Nathoo type 1 dental stains are those discolorations of the teeth caused by tea, coffee, wine, chromogenic bacteria, and metals;

  • Nathoo type 2 (N2): N2-type colored material changes color after binding to the tooth. The stains actually are N1-type food stains that darken with time; and

  • Nathoo type 3 (N3): N3-type colorless material or prechromogen binds to the tooth and undergoes a chemical reaction to cause a stain.  N3-type stains are caused by carbohydrate-rich foods (e.g., apples, potatoes), stannous fluoride, and chlorhexidine.

 

The use of dentifrices with inadequate cleaning and polishing actions is a significant factor in dental staining; but the most common determinant of extrinsic stains is poor oral hygiene.

 

 


2. Could you provide us the links to any studies for PAP & HAP. What are these studies concluding - is there a study with positive results?

3. What is exactly happening when we are whitening our teeth? Is it happening in the teeth or in the surface of the teeth? 

 

Chemically, bleaching agents including peroxides and PAP, work by breaking down into active “free radicals” that lightens the color of organic, double bonded stain molecules12 (Figure 3) that are on the surface in the subsurface pors on teeth. Bleach has no effect on other kinds of stains. However to the extent that most stains are organic and double-bonded, stains can disappear to the eye when treated with bleach for a period of time. However, its is key to note that although stains treated with bleaching agents may not visible, they actually still remain in place, and over time, re-appear as they re-acquire their original bond structure and coloring.

 

 


4. What is PAP exactly and why is it a modern way of whitening? Could you tell us all the arguments why to use PAP and not other whitening ingredients:

 

Hydrogen peroxide remains the gold standard in teeth bkeaching, because it breaks fdon into free radicals the easiest and fastest. However bleaches are orally acceptable including carbamide peroxide, sodium percarbonate and PAP.

 

PAP is the shortened name for phthalimido peroxy caproic acid (PAP), an organic peracid containing a high potential of oxidation. Oxidation is necessary for the bleaching procedure as it neutralizes organic double bonds that cause dental discolorations by releasing peroxide like free radicals.

 

 

 

 

5. How does PAP works? What is exactly happening with my teeth if I use PAP? 

 

As mentioned above PAP releases free radicals which break the carbon bonds of stain molecules,, rendering them colorless.

 

 

6. Why should I whiten my teeth self at home and not go to the dentist - what is the average price for whitening at the dentist - and what ingredients do they use ? What could be a complaint for some people to go the dentist? 

 

The main reasons to whiten your teeth at home is convenience and price. The average price for dentist teeth whitening is about $250, while at-home teeth whitening kits costs between $40 and $600. Some high-end dentist can charge a $1000 or more, while high-end teeth whitening kits can cost $100 or more. Most importantly is convenience, as most people don’t like going to the dentist and they rather do their own whitening at home. It is an 11 billion dollar industry..


7.  What are not working ingredients in whitening products that are used often that we better can avoid? Why is it better to avoid? 

 

The non-active ingredients can generally be:  glycerin (solvent), propylene glycol (solvent), triethanolamine (pH Adjuster) and carbomer (thickener). I try to not use propylene glycol, and there are other healthier thickeners.

 

8. Why is it good to have white teeth instead of yellow / brown teeth - can we link to studies where people with white teeth seems more happy or have more succes in life? 

 

People with whiter teeth are perceived to be happier, wealthier and more success. This is because teeth (a beautiful smile) are on the first things that other humans notice. Typicaly people can look as young as 13 years younger with whiter teeth.

“Social and psychological research has shown that appearance plays an important role in determining the quality of our inter- actions with others and is an important aspect of nonverbal communication.2,3 How people look can affect how they see themselves, what others think of them, and how they attract others to them. By enhancing their appearance, people can change the impression they make on others. Physical attrac- tiveness and self-evaluation have been positively correlated.2,3 Research has shown that throughout their lives attractive indi- viduals have significant advantages over those perceived by society to be less attractive.2”

 


9. Could you tell us all the negative symptoms of teeth whitening with peroxide ? How does it come it makes your teeth sensitive?

 

It has been speculated that about 25% of people who try to whiten their teeth experience considerable dental pain called dentinal hypersensitivity.t has been speculated in the dental literature that the tooth pain associated with bleaching procedures, occurs in more than two-thirds of patients undergoing bleaching treatment. The medical term for this condition is “transient dentinal hypersensitivity.” However in layman’s terms, this simply means that most patients find these treatments painful.  The nature of the pain has been described as a multi-faceted transient thermal and tactile sensitivity that occurs both during application, and lasting for 2 to 14 days beyond the end of treatment.

The exact etiology of this pain is not fully known, however the literature describes a “Hydrodynamic Theory” that can be considered to be a reasonable template for understanding.  The theory essentially explains that hypersensitivity is most probably caused by the dehydration11 of tooth enamel following a relatively long exposure to acidified and thickened peroxide gels, held against teeth in mouth trays or strips. Because bleaching works slowly, prolonged contact times with the concentrated gels result in a negative osmotic gradient. This inside-out osmotic pressure then draws the pulpal odontoblastic processes into the dentinal tubules, entrapping and pinching them as shown in the figure on the right. Ultimately, this culminates in transient dentinal pain.

10. Can we link to stories where people have ruined their teeth because of using bad formulas ? Are there maybe celebrities with ruined teeth? 

No person, no person in billions of teeth whitening procedures throughout the world has ever been permanently harmed by using  teeth whitening product. The only way you can harm your teeth is over-use of the product beyond manufacturers recommendations.

11. What other formulas are on the market and can you send us some links where we can see that it is not working so well ? Like coconut oil whitening, I cannot imagine that works well or does it? 

 

Any product that does not contain, hydrogen peroxide, carbamide peroxide, chloride dioxide, or PAP will not work well. A strong oxidizer is needed to whiten teeth. Most teeth whitening products work to some degree. For example, teeth whitening toothpaste can remove external stain and plaque and that lightens teeth about 2 shades. OTC teeth whiteners generally give 4-6 shadses an professional dental teeth bleaching whites 7-8 shades on average.



12. Why should we not use charcoal for example - are there negative effects of using charcoal?

 

Charcoal per se, cannot whiten teeth any more than any other toothpaste, however it does stain the gums darker giving the impression of whiter teeth due to the color contrast.

 


13.  Do we need led-light for the PAP whitening ? And is it a benefit not to use light? Is there some negative thing we can say about led light ? 

 

No LED light is needed for PAP teeth whitening. LED’s have no ability to enhance teth whitening, other than holding lips out of the way. Only UV-C light can cause the production of free radicals from peroxides. UV-C light is also very dangerous and only dentists should use.


14.  Any other exciting facts about teeth whitening?

 

One fact I always tell people is that teeth whitening is a life-long endeavor. Right after people people whiten their teeth they continue to eat staining foods, and these stains will have to be treated in a daily or monthly basis. IT just like your car or a white shirt, u=you don’t just clean it once. It must be cleaned very often after use. If people want permanently white teeth they should go to the dentist and get white porcelain veneers.

According to the EU government we need to stop using Peroxide because o they say it can cause cancer in mice.


According to the American Dental Association  - we need to beware of charcoal because it is very abrasive and can permanently scratch teeth.


According to Dr. Martin Giniger whitening products do not work if they are too thick to penetrate teeth. Less thick gels penetrate better,
